Client questionnaire for online booking
Create service questions, collect answers, and include extra appointment time
Why use a questionnaire

How to create a question
- Open the client questionnaire section
- Add a new question
- Enter short, clear wording
- Choose the answer type
- Decide whether the answer is required
- Link the question to the relevant services
- Add time before or after the visit when needed
- Save and make a test booking

- Select the appointment
- Open the appointment details
- Find the client questionnaire section
- Review the answers before the service begins
Recommendations
Ask only what matters
A shorter questionnaire is easier to complete
Link to services
Do not show questions unrelated to the client's order
Privacy
Do not request sensitive data unless it is genuinely necessary
Test it
Complete the questionnaire as a client after every change
The questionnaire should improve the visit
Ask questions only when the answers affect preparation, duration, or the safe delivery of the service.
